I'm using mine with my 27" iMac. You just need the USB port to charge it and a cd drive to install the driver and you're good.
I got it to scan traditional barcodes from my samsungs5 screen. But im pretty sure it will not read any of the 3d barcodes at all on the phone or on paper.
You just have to program the scanner using the manual that came with it. There will be a barcode to enter programming mode and one to end programming mode as well. In between scanning those 2 barcodes you scan other barcodes to change the behavior of the scanner.

I don't think it would be a good idea for a child. For one thing, it's a real laser, and you wouldn't want anyone shining it in their eyes, or a pet's eye. Another thing, you have to hook it up to your computer, and that would be a problem. It would show the bar codes that are scanned on a word program, or some other application. They do have toy scanners out there. There use to be a Barbie one attached to a shopping cart, it worked great, but it only scanned the bar codes on the items it came with. Honestly, for a child, I would look for a toy scanner. Hope this helped!
If you're talking about a retail-style PS/2 keyboard wedge with a phone jack (ethernet) looking thing (pictured here http://images17.newegg.com/is/image/newegg/28-997-266-TS?$S300$), no it does not. It comes with an ethernet to usb cable only, so if you'd like it to connect to an older ps/2 port on the keyboard + PC box you'll have to get it separately.
